Kids Registration Online Form

 

To register your child, please do one of the following:

1. Click on the below link to open the registration form. DO NOT ENTER ANY INFORMATION WHILE ONLINE. Instead, save the PDF file to your computer then open the file from your computer, now type in the information. Save the file again to your computer or device. Email the file to Mary or Print and turn in to front desk.

2. Click on the below link to open the registration file. Print the file. Fill out the form by hand and turn in to the front desk.

Note: You can only fill out the form from your computer if you have adobe acrabat DC. if you are not able to fill the form out online please print and fill out by hand and turn the completed form to the front desk. If you’d like to email the form, please scan the completed form and email.
